  4. Intermediate Certificate
  5. All years of your D. Pharm/B. Pharm mark sheets – merged into a single PDF file.

  7. Internship Letter (only for D.Pharm students)

  11. An Affidavit on a Rs. 100 stamp paper

A Quick Note on the Affidavit:

You will need to contact a lawyer for this. They will usually charge Rs200-Rs300 to buy the stamp paper and put their signature and seal on it. The affidavit needs to have your name, your father’s name, your college’s name, your course, and your passing year. D.Pharm students also need to add their training dates.

How to Get Your College Approval Letter:

Just search for PCI on Google, go to the official PCI website, and in the approved colleges section, click on your course (D.Pharm / B.Pharm / Pharm.D). Then, search for your college’s name or PCI code, take a screenshot, and convert it into a PDF. That’s your college approval letter.
